NATIONAL BLUE BERET FORM PACKET
The link is provided in the Welcome Email.
The packet contains all the forms needed CAPF 60-81, CAPF 161, CAPF 163
- Download the form.
- Open it with a PDF reader. Adobe Acrobat is required to view PDF files on this page, download Acrobat Reader for free from the Adobe website.
- The form fields auto-populate, ie., fill in the name field once and it automatically fills in all of the name fields on the remaining pages.
- The NBB-specific, form packet is the only CAP form that will be accepted.
- Your squadron and wing commander's signatures are not required.
- Handwritten forms will not be accepted.
Name your forms as follows.
Last name space CAPID, i.e. Smith 123456, then:

Sign (Your own)

Parent's signature (if you are a minor).

Scan and save all pages as one PDF document. This is the only format accepted. If you have trouble with this, ask for help at admin@nbb.cap.gov

Scan the front and back of your insurance card (if you have one) and include it in the pdf.
